In 2010 “The Patient Protection and Affordable Care Act” was passed and stated that all 501(c)(3) hospitals (not-for-profit or non-profit hospitals) must conduct a community health needs assessment process every three years to meet the U.S. Department of Treasury and Internal Revenue Service (IRS) rules.
It is now time for Central Community Hospital (CCH) to refresh their Community Health Needs Assessment and three-year health improvement plans. At the same time, Clayton County Public Health (CCPH) must also complete this same process every five years for Clayton County. So, CCH and CCPH have partnered together to develop a survey that we are asking Clayton County residents to fill out so that we can obtain data to help develop our health improvement action plans for the next 3-5 years.
A large group of stakeholders that contribute to the overall community health (physical, emotional, spiritual, financial) of Clayton County will be meeting at the end of May to review the results, prioritize health priorities and develop action plans for the future.
Please take moment to fill out this quick, 5-10 minute survey! The survey will be open until, May 25, 2018.
